Objectives

Family Mosaic are a leading Housing Association providing good quality, affordable homes to rent and buy, as well as care and support services to thousands of people who need extra support. 

We provide a range of opportunities for our customers such as training, employment and access to learning so they can improve their prospects, find new opportunities and move on.

Within this remit, in 2012 Family Mosaic pledged to save the NHS a substantial amount of money by helping to improve the lives of their over 50’s tenants. We embarked on a research project to find out if landlords can have a positive impact on the health and wellbeing of their over 50’s tenants, through direct one-to-one support from the health and wellbeing team.

Now, this research has been extended into a new area of London - Southwark, Lambeth and Lewisham using the Patient Activation Measure (PAM) and Family Mosaic’s Well Homes Assessment (WHA) to empower our over 50s customers to take control of their own health.

This is pioneering research in this field which we are hoping will form the way housing associations work with customers to improve lives, homes and health.

Activities

We provide care and support to over 8,000 people in London, Essex and the Southeast, to help them live more independently.

We treat every person as an individual, creating a support plan just for them, so that they live more independently, with more dignity, more choice and more control. Our personalised approach enables people to choose their own package of care: it’s been particularly appealing for younger people. And our initiative to help care and support customers get into work has been recognised nationally - and resulted in some of them finding work with us.

Under the remit of the research project we will work directly with people to encourage and motivate them to take control of their health. We will flag up vulnerable cases and ensure the appropriate referrals are made so that person receives the care and support they need.
