Objectives

Community Connect is a social prescribing service that uses a community approach to improve people’s health and well-being.  Our aim is to connect people to local services and organisations to reduce repeat appointments demand at surgeries.

 

If you have an interest in promoting health and wellbeing, a desire to help, possess good listening skills and want to develop your own skills and experience, then you may be the perfect volunteer for us!

 

Role Purpose:

  • Promote local services and organisations within the borough
  • Engage with and empower people to enable them to improve their well-being
  • Inform patients of associated benefits of connecting to the community
  • Work in partnership with local organisations to support initiatives and
  • Increase uptake of patients being referred to Community Connect
  • Reduce demand and repeat visits of patients to GP surgeries
  • Encourage a positive attitude towards health and well-being

Activities

Main Tasks:

  • Make contact with patients in a GP surgery
  • Listen to and consider individual needs in order that we may signpost them to the most appropriate service
  • Promote and encourage patients to engage with the Community Connect project
  • Complete paper and electronic referral forms
  • Maintain regular contact with the Volunteer Coordinator at BVSC
